What Does Cencora Do for Drug Prices?
Cencora does not set drug prices. Manufacturers set them, insurers and pharmacy managers negotiate them, and Cencora is the wholesaler in the middle: it buys drugs, stores and delivers them to pharmacies, health systems and doctors, and earns a small margin for doing so.
- In its fiscal third quarter of 2026 it had $84.8 billion of revenue and $1.24 billion of adjusted operating income, a 1.46% margin.
- That is why price changes matter less to its profit than to its revenue: manufacturer price cuts lowered revenue by about $2.4 billion while operating income rose 17%.
- Its profit is driven by volume, specialty drugs and services.

What does Cencora do?
Cencora (formerly AmerisourceBergen) is one of the three large US drug wholesalers. It buys medicines from manufacturers, warehouses them and delivers to thousands of pharmacies, hospitals and physician practices, and it provides services around them, such as specialty drug distribution, oncology practice support through the OneOncology platform it acquired on February 2, 2026, and animal health through its MWI business. A pharmacy that buys from Cencora gets one order, one delivery and one invoice instead of dealing with hundreds of manufacturers.
| Segment (fiscal Q3 2026) | Revenue | Operating income | Margin |
|---|---|---|---|
| U.S. Healthcare Solutions | $74.86 billion (+4.9%) | $966 million (+15.9%) | 1.29% |
| International Healthcare Solutions | $7.68 billion (+5.9%) | $166 million (+20.8%) | 2.16% |
| Other (animal health and others) | $2.25 billion (+6.9%) | $109 million (+24.8%) | 4.83% |
| Total | $84.75 billion (+5.1%) | $1.12 billion GAAP, $1.24 billion adjusted | 1.32% GAAP, 1.46% adjusted |
Operating income as a percentage of revenue by segment, from Cencora's August 5, 2026 release. The big business earns the thinnest margin.
Does Cencora control drug prices?
No. Cencora earns on the margin and services it adds, and its results show how prices and profit can move apart. In the quarter, lower manufacturer prices on some brand drugs cut about $2.4 billion from revenue (a figure from Investing.com's coverage), yet U.S. operating income rose 15.9%, helped by specialty sales and the OneOncology acquisition. GLP-1 drugs add large revenue but carry lower margins. Management said policymakers appear focused on protecting physician reimbursement and patient access.
| What changes | Effect on Cencora |
|---|---|
| A manufacturer cuts a list price | Revenue falls, and profit depends on the distribution margin and fees rather than the price |
| Spending shifts to specialty and oncology drugs | Higher-margin services and fees, as with OneOncology |
| GLP-1 drug volume rises | More revenue at lower margin |
| A large customer moves business | Volume and profit change, as with a mail-order customer and a 2025 oncology customer loss |
How did Cencora do, and what does it expect?
| Measure | Fiscal Q3 2026 | Note |
|---|---|---|
| Revenue | $84.8 billion | Up 5.1% |
| Adjusted EPS | $4.48 | Up 12.0%, against about $4.35 expected |
| Adjusted free cash flow, nine months | $1.14 billion | About $3.0 billion expected for the year |
| Share buybacks | $1.0 billion in the quarter | Completed the amount planned for the calendar year |
| Dividend | $0.60 a quarter | Paid August 31, 2026 |
| Fiscal 2026 adjusted EPS guide | $17.75 to $17.95 | Raised from $17.70 to $17.90 |
| Fiscal 2026 revenue guide | Up 4% to 6% | Adjusted operating income up 13% to 14% |
Fiscal 2027 guidance is due in November. Debt rose to about $11.7 billion after the OneOncology and Retina Consultants purchases, and net interest expense rose 72% in the quarter.
What are the risks?
- Thin margin: at about 1.5% of revenue a small shift in volume, mix or cost changes profit.
- Customer concentration: large customers such as a mail-order pharmacy can shift business, and the sources named none by share.
- Policy: drug-pricing and reimbursement rules, including 340B and Medicare Part B changes, can affect what providers and distributors earn.
- Debt: acquisitions were funded by about $3.0 billion of senior notes and $1.5 billion of term loans, so interest costs are higher.
How does COR look on this site's checks?
From the weekly data of October 23, 2026, on the Cencora stock page:
| Measure | COR | What it tells you |
|---|---|---|
| Price | $331.00 | 12% below its 52-week high of $377.54 |
| Market value | $63.2 billion | Total value of all shares |
| Trailing / forward P/E | 24.1 / 15.9 | Price per dollar of earnings, last year and expected |
| Price to sales / price to book | 0.2 / 20.7 | Revenue is huge against profit, so price to sales is near zero |
| Revenue growth / net margin | +5.1% / 0.8% | Trailing twelve months |
| Dividend yield | 0.74% | Payout ratio 12% of earnings |
| Debt to equity | 3.84 | High, partly because share buybacks have shrunk equity |
| 1-year change / RSI | +0.1% / 54.5 | RSI below 30 is oversold, above 70 is overbought |
| Value filters passed | 4 of 8 | This site's eight mechanical value filters |
| Margin of safety (DCF) | +42.7% | A neutral model that fades growth from 8% to 2.5% |
The DCF margin of safety is positive because Cencora's free cash flow per share ($33.82) is large against the price, but that number swings with working capital, so use it carefully. What is margin of safety in investing explains how to treat it.
