Is Cigna a Buy Right Now? What the Numbers Say
On this site's numbers Cigna looks inexpensive, but this site cannot tell you whether to buy it, and the discount has reasons. At $306.95 it trades at 12.9 times trailing earnings and about 10 times its 2026 adjusted EPS guidance of at least $30.45, clears all 8 of this site's mechanical value filters and shows a +50.8% margin of safety on a neutral DCF.
- Against that, its pharmacy benefit services profit fell 27% in the second quarter, the company is changing how it earns from drug rebates, and it is leaving the individual health plan business.
- The next report will show which side is winning.

What does Cigna actually do?
Cigna has two engines. Evernorth is the health services business, including Express Scripts, one of the three large pharmacy benefit managers (PBMs), which negotiate drug prices for employers and insurers, plus specialty pharmacy and care services. Cigna Healthcare is the insurer, covering about 18.4 million medical customers, mostly through employers. Evernorth is the larger earner: the company guides to at least $6.9 billion of pre-tax adjusted income from it and at least $4.55 billion from Cigna Healthcare in 2026.
Pre-tax adjusted income by business in Q2 2026, from Cigna's July 30, 2026 release. Pharmacy Benefit Services (amber) was down 27% on the year; the other two grew 17% and 22%.
How did Cigna's latest quarter go?
| Measure | Q2 2026 | A year earlier |
|---|---|---|
| Total revenue | $71.7 billion (+7%) | $67.2 billion |
| Adjusted EPS | $7.78 | $7.20 |
| GAAP EPS | $6.29 | $5.71 |

| Cigna Healthcare pre-tax adjusted income | $1,276 million (+17%) | $1,094 million |
| Cigna Healthcare medical care ratio | 84.5% | 83.2% |
The pharmacy benefit drop was attributed to client-focused initiatives, including large client contract renewals, that is, giving big customers better terms. Specialty services grew on strong organic growth. Management raised 2026 adjusted EPS guidance to at least $30.45 (up $0.10), with a medical care ratio of 83.7% to 84.7%. Shares fell about 4% on the day despite the beat, to about $284.

Cigna is the cheapest of the group on both trailing and forward earnings. It has also been flat over a year (-0.3%) while Centene (+95%), Humana (+47%) and Elevance (+27%) rose, so it has lagged its peers.
What is the case for Cigna, and what is the case against?
| The case for | The case against |
|---|---|
| Earnings are rising: adjusted EPS up 8% and guidance raised, with specialty services profit up 22% | Pharmacy benefit profit fell 27%, and Evernorth profit overall slipped 2% on large client renewals at better terms |
| Free cash flow is large: $36.54 a share, a yield near 12% of the price | Free cash flow can run ahead of earnings for a year because of working-capital timing, so check it against earnings |
| Buybacks shrink the share count, with diluted shares down 1.6% on the year | Only about $250 million was repurchased through July 29, small against an $82.6 billion market value |
| It is leaving the individual health plan business on January 1, 2027, reducing exposure to the individual plan market, which has been volatile for insurers | Customer relationships fell 3% year to date and pharmacy customers fell 4% |
| The rebate-free model could leave profit comparable while making drug pricing more transparent | It starts in 2027 for insured plans and 2028 for at least half of Evernorth clients, so the transition is ahead, with costs and risks |
| Regulatory overhang cleared in part: an FTC settlement in February 2026 carried no financial penalty | Pharmacy benefit managers remain under political and legal pressure, and reform rules take effect in 2028 |
What is the rebate-free model, and why does it matter?
PBMs have traditionally been paid partly from rebates they negotiate from drug makers, which tie their income to a drug's list price. Cigna announced in October 2025 that Express Scripts will move to a rebate-free model, with fully insured plans adopting it in 2027 and at least half of Evernorth's clients by the end of 2028, and said it expects comparable profitability between the old and new models, though the source of the profit will change. The FTC settlement of February 2026 pointed the same way: Express Scripts will be paid a per-member, per-prescription administrative fee not linked to a drug's price, and it carried no financial penalty. PBM reform signed in early February 2026 also delinks pay from rebates in Medicare Part D.
The risk is execution: new contract terms, client renewals at lower margins and the cost of building the model. Management said investments in it could weigh on near-term results.
How does CI look on this site's checks?
From the weekly data of October 23 to 24, 2026, on the Cigna stock page:
| Measure | CI | What it tells you |
|---|---|---|
| Price | $306.95 | 3% below its 52-week high of $311.34 |
| Market value | $82.6 billion | Total value of all shares |
| Trailing / forward P/E | 12.9 / 8.4 | About 10.1 times the 2026 adjusted EPS guide of $30.45 |
| Price to sales / price to book | 0.29 / 1.94 | Insurers and PBMs have thin margins, so sales multiples run low |
| Revenue growth / net margin | +7.8% / 2.3% | Trailing twelve months |
| Dividend yield / payout | 1.97% / 25% | A low payout leaves room to raise it |
| Free-cash-flow yield | 11.9% | Free cash flow per share $36.54 |
| Debt to equity | 0.75 | Total debt against shareholders' equity |
| Value filters passed | 8 of 8 | Buffett-style 5 of 8 and Lynch-style 3 of 5 |
| Margin of safety (DCF) | +50.8% | A neutral model that fades growth from 8% to 2.5%, earnings quality rated Strong |
Passing all eight filters and a +50.8% margin of safety describe a low price against current earnings and cash flow. They do not say the earnings are safe: the filters cannot see a change in how a PBM is paid.
